Score the potential severity of the Hazard(s)

NO. OF PEOPLE AT RISK

Estimate the number of people that might be injured by the hazard in the event of an accident or incident or exposure

 

SENSITIVITY OF PEOPLE AT RISK

Determine how sensitive the people at risk are to the hazard concerned (e.g. Are there any high risk groups such as pregnant women, young people, visitors, members of the public, disabled persons, night workers, lone workers, new employees, contractors not aware of the hazards ? Are workers sensitive because of their medical history - e.g. allergies, pacemakers, etc.). With manual handling activities or work in confined spaces or work requiring respirators, are workers fit to do the work? Is there health surveillance?

 

DURATION OF EXPOSURE

How often are people exposed to the hazard? For example how many hours is a person doing cleaning exposed to the chemicals used, how many hours does a forklift operate, how many hours does someone in an office work at a Computer - is it constant during working hours or just for a few hours at a time ? Is it every day or just occasionally - for example production work may be constant but maintenance may be occasional?

 

NATURE OF HAZARD

How hazardous is the hazard? What is the extent of the injury or damage that may result from an accident or exposure? Death, permanent disability, cuts, bruises. It may be hazardous because of its chemical properties (e.g. Toxic, Carcinogenic, Flammable, Corrosive, etc ) or because of its energy or potential energy ( e.g. electricity, radiation - heat, UV, ionising, moving vehicle or machine part or animal, heavy object) or because of the nature of the material (e.g. sharp edges) or because of biological agents (e.g. contaminated blood, sewage, animals)

 

REGULATORY LIMITS/GUIDELINES

Do you know if there are any Legal limits or guidelines or Company Policy associated with this hazard? Are you operating within them?

A separate pop-up window should have been displayed during the move from Step 1 to 2 which displays relevant legislation checklists based on the Hazards chosen.

 

Score the probability that this Hazard will occur

WORKPLACE/JOB DESIGN (e.g. ISOLATION, ORGANISATIONAL CONTROLS)

Have you assessed the design of the activity or workplace in relation to the hazard concerned? E.g. Ergonomics, lighting, access control or isolation of the activity from other activities to limit the hazard, safety warning signs, etc

 

ELIMINATION / REDUCTION OF RISK (e.g. ENGINEERING CONTROLS)

Have you introduced measures to eliminate or reduce the risk from the hazard? For example wearing of PPE, fitting of interlocks, fire protection systems, extraction systems, intrinsically safe electrical equipment, mechanical aids, etc

 

ADMINISTRATIVE CONTROLS (e.g. DOCUMENTATION)

Have you introduced any procedures or work instructions to increase safety in relation to the hazard? Are these adequate?

 

KNOWLEDGE/COMPETENCY

Have operators and any other people that may be affected by the hazard received training in relation to the hazard? Are they competent to carry out the activity safely? Is certified training required? (E.g. Forklift operators, First Aid, Electricians, Transport of Dangerous Goods ?)

 

WORK EQUIPMENT (INCL. PPE)

Work equipment can include PPE, Desks & Chairs, Computers, Tools or Machinery. Is PPE provided if required? Does the work equipment being used for the activity concerned meet any required standards? (E.g. is it CE marked?)

MONITORING/INSPECTIONS

Do you carry out monitoring or inspections of the activity or the people to ensure safety? (e.g. safety inspections, housekeeping inspections, occupational exposure monitoring)

 

HISTORY OF COMPLAINTS, INCIDENTS OR NEAR MISSES

Have there been any complaints, incidents or near misses involving this hazard? Were any corrective actions dealt with? Are employees expressing concerns during this assessment?

 

EQUIPMENT RELIABILITY & MAINTENANCE

Is the work equipment used in relation to the activity/hazard reliable and well maintained?

 

MODE OF WARNING

In the event of an accident or exposure to the hazard would the injury be sudden or gradual? Could exposure be planned & controlled to limit its effect? (e.g. Emergency Plans)

 

MANAGEMENT SYSTEM AUDIT

Do you carry out management system audits in relation to this activity/hazard?
